How to Join

We appreciate your interest in joining Prospect Medical Group.

Physicians should submit a letter of interest, W-9, a current Curriculum Vitae, and a completed questionnaire to our Provider Contracting Department via email.

We will review your information—along with our current network needs—and provide a response to you within 30 days. Your interest is most appreciated, and we look forward to partnering with you.

For physicians who need to update their practice information, such as address, suite, phone or fax changes, please send the updated information to us via email. Be sure to include the new change, effective date and W-9.

Provider Portal – Aerial Care Login

Aerial Care is Prospect’s provider portal that allows providers to check eligibility status, referral submission, referral status and referral modifications, claim status and access provider resources for member care coordination.

Provider Relations

Our provider relations team serves as the liaison between our providers and Prospect Medical Group. Among other things, the department educates you and/or your staff on Aerial Care, Prospect Medical's web-based physician portal, for referral submissions, our Physician Incentive Programs and customer service training. We also assist with:

  • Contracts
  • Claims
  • Authorizations
  • Provider and patient eligibility questions
