Who is Eligible

Our outpatient services are available to patients who meet one or more of the following criteria:

Eligible for outpatient Services

ü  recently discharged from the hospital or emergency department

ü  managing one or more chronic conditions (e.g., diabetes, heart failure, COPD, chronic kidney disease, asthma)

ü  at risk of frequent hospital visits or readmissions

ü  experiencing barriers to care such as transportation, housing, or access to medication

ü  referred to by a healthcare provider, case manager, or social worker

ü  covered by participating insurance plans or eligible under community-based programs

 

 

Additional Eligibility Notice:

Participation in the program is limited to patients enrolled in certain coverage plans. Please consult with your care team to verify eligibility.
