Outpatient Care Management allows patients to receive expert medical attention and support services while continuing to live at home. It is flexible and patient centered.

Who is Eligible
|
Our outpatient services are available to patients who meet one or more of the following criteria: |
|
|
|
ü recently discharged from the hospital or emergency department |
|
ü managing one or more chronic conditions (e.g., diabetes, heart failure, COPD, chronic kidney disease, asthma) |
|
|
ü at risk of frequent hospital visits or readmissions |
|
|
ü experiencing barriers to care such as transportation, housing, or access to medication |
|
|
ü referred to by a healthcare provider, case manager, or social worker |
|
|
ü covered by participating insurance plans or eligible under community-based programs |
|

Participation in the program is limited to patients enrolled in certain coverage plans. Please consult with your care team to verify eligibility.
